How Much Does Intensity-modulated Radiotherapy (IMRT) Cost in India?

The cost of Intensity-modulated radiotherapy (IMRT) in India typically ranges between USD 4000 - USD 5000.

However, this cost can vary depending on several factors, including the type and severity of the condition, treatment techniques chosen, the healthcare facility's location and reputation, the treating professionals' experience and specialisation, and the patient's overall health status.

Additionally, factors like the duration of treatment, the need for follow-up care, and the use of advanced technologies or specialised treatments can further influence the overall cost.

Factors Influencing the Cost of Intensity-modulated Radiotherapy (IMRT):

A radiation therapy method called intensity-modulated radiotherapy (IMRT) employs computer-controlled radiation to target tumours accurately while causing the least amount of harm to nearby healthy tissue. Numerous cancer forms are treated using IMRT.

IMRT uses multiple beams, varying beam strength, and customises radiation beams.

When treating tumours close to vital organs, IMRT is typically thought to be a safer alternative to traditional radiation treatments. When treating tumours close to critical organs, IMRT is normally believed to be a safer alternative to conventional radiation treatments.

Intensity-modulated radiation therapy (IMRT) stands as an advanced form of radiation therapy in cancer treatment. By utilizing potent energy beams—derived from X-rays, protons, or alternative sources—IMRT precisely targets cancer cells, sparing healthy tissue.

IMRT employs customized radiation beams, meticulously tailored to match the contours of the cancerous area. These beams possess the flexibility to move along arcs during treatment delivery, allowing for optimal coverage. Additionally, the intensity of each beam can be finely adjusted, ensuring accurate radiation dosage.

This technique enables the safe and efficient delivery of radiation therapy, minimizing damage to surrounding healthy tissues. IMRT's versatility extends to the treatment of both cancerous and benign tumors across various body regions, with the primary objective being to administer the appropriate radiation dose to the target while mitigating collateral damage.

The following types of cancer can be treated by IMRT:

  • Breast cancer
  • Cervical cancer
  • Colon cancer
  • Lung cancer
  • Prostate cancer
  • Stomach cancer
  • Uterine cancer
  • Other cancer types

Types:

  • Step-and-Shoot IMRT:( the treatment is delivered in a series of small, discrete steps.)
  • Sliding Window IMRT:(known as dynamic MLC,) This involves continuously moving the MLC leaves while the radiation beam is on, allowing for a more seamless delivery of radiation dose.
  • Volumetric Modulated Arc Therapy (VMAT):(This is an advanced form of IMRT where the linac delivers radiation continuously as it rotates around the patient.)
  • Tomotherapy:(This technique combines IMRT with computed tomography (CT) scanning.)It delivers radiation in a helical manner.

IMRT is a type of radiation therapy applied to treat cancer by providing targeted, high-dose radiation to tumors while avoiding surrounding healthy tissue. IMRT is frequently used for head and neck cancers, prostate cancer, brain tumors, breast cancer, and pelvic cancers, particularly where tumors are close to vital structures.

You need to consult an oncologist if you have a cancer diagnosis and radiation forms part of your treatment. IMRT may be advised when standard radiotherapy carries a high risk to surrounding organs, or when high-precision treatment is required to optimise outcomes and minimise side effects.

Before initiating IMRT, you will have imaging tests (CT, MRI, or PET scans) for planning. Minor skin marks or a custom mask or body mould can be created to help keep your body in place during treatment. The radiation oncologist will discuss the schedule, dietary recommendations, and what to anticipate.

Imaging scans evaluate the size and shape of the tumor. They are delivered in a daily session (fraction) over a few weeks. Each session involves lying still on a table while the machine rotates around you, aiming at the tumor from many different angles.

Each IMRT treatment session lasts 10–30 minutes, although most of that time is spent on setup and alignment. Treatment may take 5 to 7 weeks and is given 5 days per week.

  • Fatigue
  • Skin irritation
  • Hair loss at the site
  • Nausea
  • Dry mouth
  • Diarrhea
  • Urinary problems

IMRT provides accurate tumor targeting, enabling higher doses of radiation with less damage to normal tissues, lowering side effects and enhancing quality of life. It is beneficial for complicated or difficult-to-reach tumors.

There is no post-operative recovery as it is non-surgical. Side effects will accumulate over time and persist for several weeks after therapy completion. Patients are monitored regularly and may require medications, skin care, or nutritional assistance during and after therapy.

IMRT is reported to have high success rates, between 75% and 95% for tumor control and survival improvement in most forms of cancer. Outcomes depend on the type of cancer, stage, and overall health, but are usually better than traditional radiotherapy when accuracy is essential.

Process Involved for Intensity-modulated radiotherapy (IMRT) in India

  • Consultation: The radiation oncologist or oncologist will discuss the cancer diagnosis and tell the patient the advantages of IMRT and its position in the treatment plan.
  • Pre-treatment Evaluation: Imaging studies are done to establish the precise location, size, and form of the tumor, along with the surrounding tissues that must be spared.
  • Treatment Planning: A specialised treatment plan is developed through extensive 3D imaging to target the tumor accurately and with as minor damage as possible to healthy tissues.
  • Administration of IMRT: IMRT is administered daily or after several weeks. Each treatment is precisely calibrated to deliver radiation directly to the tumor.
  • Post-treatment Monitoring: Regular follow-up appointments are made to monitor the response to treatment, for side effects, and for determining the long-term efficacy in suppressing or shrinking the tumor.
  • Head and neck cancers
  • Prostate cancer
  • Brain tumors
  • Gastrointestinal cancers
  • Sarcomas
  • Certain breast cancers
  • Lung cancer and other cancer patients are recommended to undergo IMRT if precise targeting of the tumor is crucial. The type, site, and extent of the cancer, the general health of the patient, and any other underlying medical conditions are considered before making the decision.
  • When a tumor is located near critical structures that need to be protected from radiation, IMRT is typically used.
  • Radiation therapy
  • Concurrent therapies (chemotherapy, immunotherapy, or surgery)
  • Improves tumor control with fewer side effects, enhancing the patient's quality of life during and after treatment.

Radiotherapy is a type of treatment given in a cancer treatment plan. It can be used in early and advanced stages, where cancer spreads. Its goals are:

  • To shrink or cure early-stage cancers: Some cancers are sensitive to radiation and may go away completely or shrink due to this. Before giving radiation, specific anti-cancerous drugs and chemotherapy may be given or may be combined together.
  • To stop the recurrence of cancer: Cancer may spread or metastasize to other body parts from where it starts. Radiation is used to destroy these before they develop into tumors, even before they are visible on MRI and CT scans.
  • To treat symptoms of advanced cancer: Sometimes cancer spreads extensively and cannot be cured, but it is still possible to make the tumors smaller. It can help in relieving symptoms like trouble breathing or swallowing, pain, bowel changes, etc.

There are several preparation steps before undergoing radiotherapy:

  • One should talk to their radiation oncologist about their expectations: It is not uncommon for people to be surprised by how radiation affects them. The effects of radiation are less visible, unlike hair loss and scars with chemotherapy and surgery. Ways to prevent skin rashes and redness, and what personal care products to avoid.
  • One should talk to their radiation oncologist about respiratory gating: Radiation has been linked with various forms of heart diseases like rhythm disturbances, valve disease, coronary artery disease, etc. This can be reduced through deep-inspiration breath-hold (DIBH) and respiratory gating. DIBH involves taking deep breaths in and then holding it while the radiation is going on. Respiratory gating synchronizes the radiation doses to one’s breathing for dose delivery when the tumor is farthest from the heart. It is important to ask about these techniques before the process starts as special measurements will be taken to ensure that the inhaled air in the lungs moves the heart away from the heart.
  • One should streamline work and home responsibilities: Many patients are able to balance their work and home responsibilities even with their radiation therapy but it can become challenging.
