Debridement of the Achilles tendon is a surgical procedure used to treat chronic tendon problems, such as Achilles tendinitis or tendinosis, caused by repetitive stress or injury. It is commonly performed when the tendon has suffered significant damage, including scar tissue buildup or degeneration, interfering with its ability to heal naturally.

The surgery involves carefully removing the damaged tissue from the tendon. Sometimes, the surgeon may remove any bone spurs or other growths affecting tendon movement.

The goal is to reduce pain, promote healing, and improve the tendon’s function. If the tendon is severely damaged, grafts may be used to strengthen and repair the tendon, promoting better healing. After the procedure, the patient may be required to wear a boot or use crutches to avoid putting stress on the tendon while it recovers. Physical therapy is also recommended to help regain strength, flexibility, and mobility. The surgery aims to reduce pain, restore tendon function, and enable the patient to return to normal activities, such as walking, running, or playing sports, with better mobility and less discomfort.

Debridement of the Achilles tendon is performed to treat Achilles tendinopathy or chronic inflammation, especially when there are degenerative or torn fibers, calcifications, or scar tissue buildup that cause persistent pain, swelling, and weakness. It is recommended when non-surgical treatments (like physical therapy, rest, or medications) have failed to improve symptoms over 3–6 months. The goal is to remove damaged tissue, promote healthy tendon healing, and restore strength and mobility.

Debridement of the Achilles tendon typically takes 45 to 90 minutes and is performed under general or regional anaesthesia.

Incision & Exposure: To reach the tendon, a little incision is made across the rear of the ankle. Tendon cleaning, also known as debridement, involves carefully removing any calcified, scarred, or degenerated tendon.
Repair or Augmentation (if necessary): To preserve strength, the tendon may be strengthened with sutures or a graft (from adjacent tissue or synthetic material) if a considerable amount of tissue is removed. Minimally Invasive Option: For a faster recovery and minimal scarring, arthroscopic or percutaneous procedures may be employed in some situations.

The surgery lasts around 1 to 1.5 hours. Most patients go home the same day. Initial non-weight-bearing may last 2–3 weeks, followed by gradual return to full activity over three to six months, depending on the extent of damage and repair.

Return to sports or high-impact activities may be expected around 4 to 6 months, depending on healing and rehab progress.

Achilles tendon debridement has a success rate of about 70–90%, with most patients gaining significant pain relief and functional improvement. Recovery is gradual—light activity by 3–4 months and full return to sport by 6–9 months with proper rehab.
