Ankle arthroscopy is a surgery that enables surgeons to look at, diagnose, and treat ailments within the joints of the ankle with the assistance of a pencil-sized camera named an arthroscope. Most often, surgeons recommend it to patients with chronically painful, unstable ankle or joint injuries who have not improved with outpatient treatment.

Ankle arthroscopy is a minimally invasive surgical procedure used to diagnose and treat problems within the ankle joint. It allows surgeons to visualize, assess, and treat conditions such as ankle impingement, ligament injuries, loose bone or cartilage fragments, synovitis, arthritis, and cartilage damage. By addressing these issues early, ankle arthroscopy helps reduce pain, restore joint function, and prevent long-term degeneration.

You should consult an orthopedic or foot and ankle specialist if you experience persistent ankle pain, swelling, instability, limited motion, or locking and catching sensations that don’t improve with rest, physiotherapy, or medication. Repeated ankle sprains, unresolved sports injuries, or joint stiffness following trauma may also require further evaluation and could indicate the need for arthroscopy.

The first steps in preparation include a clinical examination, MRI, X-rays, and occasionally CT scans to evaluate the joint. Upon scheduling surgery, patients are advised to fast for 6–8 hours before the procedure. Tell the surgeon about any allergies, conditions, or drugs you are currently using. Additionally, anesthetic evaluations and blood tests may be carried out. Preoperative education covers topics such as rehabilitation, mobility aids (such as crutches), and post-operative care.

Ankle arthroscopies are typically performed under either general or regional anesthesia:

Methods: A tiny camera (arthroscope) and surgical tools are placed through small incisions (portals) created around the ankle. Using a monitor to view the inside of the joint, the surgeon removes or fixes any loose bodies, bone spurs, or damaged tissue.

  • Common procedures performed arthroscopically include:
    • Removal of scar tissue or inflamed synovium
    • Repair of torn ligaments or cartilage
    • Debridement of arthritis or impingement lesions
    • Treatment of osteochondral defects

The procedure typically lasts 30 minutes to 1.5 hours, depending on the extent of the condition. Most patients go home the same day. In more complex cases or if combined with other procedures, a short hospital stay may be needed.

The success rate of ankle arthroscopy is 85–95%, especially when it is done for moderate arthritis, loose bodies, and impingement. The majority of patients report reduced pain and better joint function, which allows them to resume regular activities or sports with the right treatment and therapy.
