An advanced form of radiation therapy is called intensity-modulated radiation therapy, or IMRT. Strong energy beams are used in radiation therapy to destroy cancer cells. Protons, X-rays, and other sources are possible sources of energy.

Types:

  • Step-and-Shoot IMRT:( the treatment is delivered in a series of small, discrete steps.)
  • Sliding Window IMRT:(known as dynamic MLC,) This involves continuously moving the MLC leaves while the radiation beam is on, allowing for a more seamless delivery of radiation dose.
  • Volumetric Modulated Arc Therapy (VMAT):(This is an advanced form of IMRT where the linac delivers radiation continuously as it rotates around the patient.)
  • Tomotherapy:(This technique combines IMRT with computed tomography (CT) scanning.)It delivers radiation in a helical manner.

IMRT is a type of radiation therapy applied to treat cancer by providing targeted, high-dose radiation to tumors while avoiding surrounding healthy tissue. IMRT is frequently used for head and neck cancers, prostate cancer, brain tumors, breast cancer, and pelvic cancers, particularly where tumors are close to vital structures.

You need to consult an oncologist if you have a cancer diagnosis and radiation forms part of your treatment. IMRT may be advised when standard radiotherapy carries a high risk to surrounding organs, or when high-precision treatment is required to optimise outcomes and minimise side effects.

Before initiating IMRT, you will have imaging tests (CT, MRI, or PET scans) for planning. Minor skin marks or a custom mask or body mould can be created to help keep your body in place during treatment. The radiation oncologist will discuss the schedule, dietary recommendations, and what to anticipate.

Imaging scans evaluate the size and shape of the tumor. They are delivered in a daily session (fraction) over a few weeks. Each session involves lying still on a table while the machine rotates around you, aiming at the tumor from many different angles.

Each IMRT treatment session lasts 10–30 minutes, although most of that time is spent on setup and alignment. Treatment may take 5 to 7 weeks and is given 5 days per week.

There is no post-operative recovery as it is non-surgical. Side effects will accumulate over time and persist for several weeks after therapy completion. Patients are monitored regularly and may require medications, skin care, or nutritional assistance during and after therapy.

IMRT is reported to have high success rates, between 75% and 95% for tumor control and survival improvement in most forms of cancer. Outcomes depend on the type of cancer, stage, and overall health, but are usually better than traditional radiotherapy when accuracy is essential.

Process Involved for Intensity-modulated radiotherapy (IMRT)
