POEM is performed to relieve swallowing difficulties caused by achalasia and other esophageal motility disorders. The esophagus becomes constricted due to tight muscles, and it is necessary to cut the tight muscle(s) so that food can pass through.

Preparation for POEM includes: A liquid diet the day before your appointment, fasting prior to the procedure, discussing your medications (particularly if you take blood-thinner medications), and having your doctor perform a pre-operative evaluation.

The POEM procedure is performed under general anesthesia using endoscopy (a flexible scope introduced through the mouth). The doctor creates a small tunnel in the wall of your esophagus through which muscle fibres are cut at the lower esophageal sphincter. Cutting these muscle fibres relieves the constriction associated with achalasia.

Typically, POEM takes 1-3 hours. The procedure takes longer depending on the complexity.
