Endoscopic retrograde cholangiopancreatography (ERCP) is a procedure utilized for diagnosing and treating issues in the liver, gallbladder, bile ducts, and pancreas. The causes of issues that necessitate an ERCP procedure primarily stem from narrowing or blockage of the bile ducts and pancreatic ducts. One common cause is the formation of gallstones, which can become lodged in the common bile duct, often requiring surgical intervention. Gallstones predominantly consist of cholesterol, although in approximately 20% of cases, they are composed of calcium and pigments such as bilirubin. Factors such as dietary imbalance and irregular lifestyle can contribute to the development of infections. Additionally, habits like alcohol consumption and a diet high in fatty foods can lead to the onset of acute and chronic pancreatitis.
ERCP aims to detect and treat problems with the pancreas, gallbladder, and bile ducts. Within the biliary or pancreatic system, it is commonly utilised to extract stones from the bile ducts, clear blocked-up ducts, obtain tissue samples (biopsies), place stents, and treat blockages or tumours.
You must see a doctor if you have recurring episodes of pancreatitis, pale stools, dark urine, jaundice, or ongoing upper belly pain. These can be related to a pancreatic condition requiring further evaluation or bile duct obstruction.
Fasting for at least 6 to 8 hours before surgery is included in the preparation. Blood tests, imaging tests, and review of medications may be done on you. Blood thinners may need to be discontinued. Your care team will discuss sedation risks and allergies in advance.

Post-operatively, patients can experience moderate bloating or throat pain. Light activity for 24 hours is recommended. Follow-up may include imaging, laboratory tests, and stent follow-up if a stent was placed. Depending on the outcome, diet and medication may be altered.
ERCP success rates are between 85 and 95%, depending on the indication. Imaging-guided procedures performed by experienced hands yield the best results. Although rare, complications can be treated with early intervention.
