The procedure known as endoscopic retrograde cholangiopancreatography, or ERCP, is used to identify and treat issues with the pancreas, liver, gallbladder, and bile ducts. It combines the use of an endoscope—a long, flexible, illuminated tube—with X-ray technology. The scope is guided by your healthcare professional via your mouth and neck, esophagus, stomach, and duodenum, the first portion of the small intestine. Your medical professional can look inside these organs to see if there are any issues. He or she will next insert a dye into a tube and pass it through the scope. This makes the X-ray organs more visible.

Endoscopic retrograde cholangiopancreatography (ERCP) is a procedure utilized for diagnosing and treating issues in the liver, gallbladder, bile ducts, and pancreas. It integrates X-ray imaging with the assistance of an endoscope—a lengthy, flexible tube equipped with a light. Your healthcare provider guides the endoscope through your mouth, throat, esophagus, and stomach, and into the first part of the small intestine (duodenum). This enables doctors to visually inspect the internal organs and identify any abnormalities. Following this, a dye is injected through a tube passed through the scope, which helps highlight the organs during X-ray examination.

Why might one need ERCP?

To determine the source of unexplained stomach pain or skin and eye yellowing (jaundice), an ERCP may be necessary. It may be used to obtain further information if you have pancreatitis or cancer of the liver, pancreas, or bile ducts.

Additional components that may be found with ERCP include:

  • Blockages or stones in the bile ducts
  • Fluid leakage from the bile or pancreatic ducts
  • Blockages or narrowing of the pancreatic ducts
  • Tumors
  • Infection in the bile ducts

causes:

The causes of issues that necessitate an ERCP procedure primarily stem from narrowing or blockage of the bile ducts and pancreatic ducts. One common cause is the formation of gallstones, which can become lodged in the common bile duct, often requiring surgical intervention. Gallstones predominantly consist of cholesterol, although in approximately 20% of cases, they are composed of calcium and pigments such as bilirubin. Factors such as dietary imbalance and irregular lifestyle can contribute to the development of infections. Additionally, habits like alcohol consumption and a diet high in fatty foods can lead to the onset of acute and chronic pancreatitis.

ERCP aims to detect and treat problems with the pancreas, gallbladder, and bile ducts. Within the biliary or pancreatic system, it is commonly utilised to extract stones from the bile ducts, clear blocked-up ducts, obtain tissue samples (biopsies), place stents, and treat blockages or tumours.

You must see a doctor if you have recurring episodes of pancreatitis, pale stools, dark urine, jaundice, or ongoing upper belly pain. These can be related to a pancreatic condition requiring further evaluation or bile duct obstruction.

Fasting for at least 6 to 8 hours before surgery is included in the preparation. Blood tests, imaging tests, and review of medications may be done on you. Blood thinners may need to be discontinued. Your care team will discuss sedation risks and allergies in advance.

  • Preparation: The patient fasts and is given anaesthesia or sedation.
  • Insertion of the Scope: An endoscope is inserted into the duodenum through the mouth.
  • Locate the Ampulla: The opening of the pancreatic and bile ducts is here.
  • Dye Injection: Contrast dye is injected to view the ducts on an X-ray.
  • Treatment involves biopsies, stent placement, removal of the obstruction, and stone removal.

Typically, the procedure lasts between thirty and ninety minutes. Depending on their status, some patients must stay overnight, though most are observed for a few hours afterwards and can be discharged the same day.

  • Infection of Pancreatitis
  • Bleeding
  • An intestinal perforation
  • An allergic response to anaesthetic or dye

ERCP allows doctors to diagnose and treat patients immediately in a single operation. It may relieve symptoms, eradicate or prevent infections, remove blockages, and avoid more invasive surgery.

Post-operatively, patients can experience moderate bloating or throat pain. Light activity for 24 hours is recommended. Follow-up may include imaging, laboratory tests, and stent follow-up if a stent was placed. Depending on the outcome, diet and medication may be altered.

ERCP success rates are between 85 and 95%, depending on the indication. Imaging-guided procedures performed by experienced hands yield the best results. Although rare, complications can be treated with early intervention.
