By eliminating extra prostate tissue that obstructs urine flow, Holmium Laser Enucleation of the Prostate (HoLEP) improves bladder emptying and relieves urinary symptoms associated with benign prostatic hyperplasia (BPH).

Your doctor will review your medical history and current prescriptions in the weeks leading up to HoLEP and may recommend discontinuing certain medications, including blood thinners, to reduce the risk of bleeding. The size and form of the prostate are evaluated through a physical examination, which includes a rectal exam. To assess the bladder and prostate, preoperative testing, including blood and urine tests and imaging investigations like CT, MRI, or ultrasound, may be requested. Additionally, your doctor will provide precise pre-surgery instructions and review anaesthesia options, such as spinal or general anaesthesia.
HoLEP is performed under general or spinal anaesthesia, and antibiotics may be administered to reduce the risk of infection. The patient is placed on their back with their legs raised once anaesthesia has taken hold. To see the prostate, the surgeon places a resectoscope into the urethra. The enlarged prostate tissue that is obstructing urine flow and closing blood vessels is then carefully sliced and removed using a holmium laser. A morcellator is used to break up and suck out the excised tissue, and samples are sent for analysis in a lab. According to the procedure, a urinary catheter is inserted to facilitate healing, and the instruments are removed.
Holmium Laser Enucleation of the Prostate (HoLEP) typically takes 1 to 3 hours, depending on prostate size and the complexity of the condition. The exact duration may vary from patient to patient based on individual clinical factors.

Following HoLEP, most patients either return home the same day or have a brief hospital stay with a temporary catheter, which is typically removed the same day. Initial symptoms include mild pain, burning when urinating, frequent urination, urgency, and blood in the urine. These symptoms eventually improve with medication and fluids. It may take many weeks to months to fully recover and experience a reduction in urinary symptoms; pelvic floor exercises and follow-up appointments can aid in this process.
Holmium Laser Enucleation of the Prostate (HoLEP), a minimally invasive prostate surgery, is a "gold standard" for Benign Prostatic Hyperplasia (BPH), even for large prostates, with very high success rates, frequently exceeding 90-95% in improving urinary flow and symptoms, excellent long-term durability, and low retreatment rates. For most patients, HoLEP provides significant symptom relief and catheter independence.
