Total hip replacement surgery also called total hip arthroplasty, is a surgical procedure conducted to replace the damaged or diseased hip joint with an artificial joint or prosthesis. The hip prosthesis contains the following three components:

  • A stem, which fits into the thigh bone
  • A ball that fits into the stem
  • A cup which is inserted into the socket of the hip joint

The two types of prostheses used in total hip replacement surgery include a.) cemented and b.) uncemented prostheses. A combination of both is sometimes used during the surgery, depending on the recommendation made by the patient.

Hip joint surgery is conducted in patients when other non-surgical and surgical alternatives fail to work and relieve the patient of terrible hip pain. A successful hip joint surgery ensures increased mobility, improved function of the hip joint, and pain-free movement.

Two types of total hip replacement surgery differ based on whether a single or both hip joints are replaced. Total hip replacement bilateral refers to the replacement of joints on both sides of the hips. This procedure is conducted when both sides are affected.

The following are the conditions that require hip replacement surgery:

  • Osteoarthritis: Commonly called wear-and-tear arthritis, it gradually damages the slick cartilage that covers the ends of bones and helps joints move smoothly.
  • Rheumatoid arthritis: It is triggered by an overactive immune system, including inflammation that can erode cartilage and, at times the underlying bone, resulting in damaged and deformed joints.
  • Osteonecrosis: This condition occurs by the insufficient blood supply to the ball portion of the hip joint, and might result from dislocation or fracture that leads to deformities.

Total hip replacement is done to restore function, ease pain, and enhance the quality of life in individuals suffering from severe hip joint destruction. The causes include osteoarthritis, rheumatoid arthritis, fractures, or other degenerative joint disorders. The aims are to restore function, decrease pain, and enhance the quality of life.

You need to consult a doctor if you have chronic hip pain, stiffness, walking difficulty, restricted range of motion, or if conservative measures (medications, physical therapy) are no longer working. X-rays or MRI can verify the degree of joint damage.

Preparation involves medical assessment, imaging (X-rays, MRI), blood work, and occasionally cardiac clearance. Some medications may need to be discontinued quickly prior to surgery, and home prepared for post-op mobility. Pre-surgical physical therapy is sometimes advised.

  • Incision: Depending on the approach used, an incision is made over the hip joint, either anterior, lateral, or posterior.
  • Removal of Damaged Cartilage and Bone: The surgeon removes the damaged femoral head (hip joint ball) and cleans the socket (acetabulum) of any affected cartilage and bone.
  • Implanting the Prosthetic Socket: A new cup of metal or ceramic is implanted into the ready hip socket, usually lined with a plastic or ceramic spacer for smooth gliding.
  • Inserting the Femoral Component: A metal stem is inserted into the femur (thighbone), and a metal or ceramic ball is implanted on top of the stem as the new joint head.
  • Securing the Implants: The parts are secured in position with bone cement or by press-fit techniques which enable natural bone to grow into the implant as time passes.
  • Closure: The incision is closed with sutures or staples, and a dressing is applied.

Surgery usually takes 1–2 hours. Hospitalisation is typically 1–3 days, depending on recovery and the patient's health. Some patients are eligible for discharge on the same day.
