POEM is performed to relieve swallowing difficulties caused by achalasia and other esophageal motility disorders. The esophagus becomes constricted due to tight muscles, and it is necessary to cut the tight muscle(s) so that food can pass through.
If you continue to have difficulty swallowing, experience regurgitation, have chest pain, or are losing weight, you should consult your physician to determine whether treatment options such as POEM are indicated.
Preparation for POEM includes: A liquid diet the day before your appointment, fasting prior to the procedure, discussing your medications (particularly if you take blood-thinner medications), and having your doctor perform a pre-operative evaluation.
The POEM procedure is performed under general anesthesia using endoscopy (a flexible scope introduced through the mouth). The doctor creates a small tunnel in the wall of your esophagus through which muscle fibres are cut at the lower esophageal sphincter. Cutting these muscle fibres relieves the constriction associated with achalasia.
Typically, POEM takes 1-3 hours. The procedure takes longer depending on the complexity.

It includes minimally invasive procedures with no external incisions, effective relief of dysphagia, shorter hospital stay, quicker recovery, and fewer complications compared to traditional open surgery.
Patients are monitored closely after the procedure as they begin to recover from anesthesia. Patients typically spend one day in the recovery room; they are initially placed on a liquid diet and can advance to soft foods within a few days. Most patients return to normal activities within days to a few weeks. Follow-up care and dietary recommendations will be provided during this time.
POEM (Peroral Endoscopic Myotomy) for achalasia shows high success rates, generally ranging from 82% to over 95% for long-term symptom relief, though some variation exists depending on follow-up time and patient groups.

- The Preoperative Evaluation: Before the procedure, the patient has an endoscopy, esophageal manometry, imaging, and bloodwork performed. It is recommended that the patient abstain from food and that medication be adjusted as needed before the procedure.
- The Procedure: After the patient receives general anesthesia, the surgeon uses an endoscope that is passed through the mouth to make a tunnel in the esophagus. The surgeon will carefully cut the very tight muscle fibres of the esophagus to create an opening that allows the patient to swallow more easily.
- Post-Procedure: The patient will be monitored for any complications and be placed on a liquid diet. After approximately three days, the patient will gradually progress to a soft diet.
