One type of surgical treatment for epilepsy is a corpus callosotomy. Recurrent seizures are a long-term (chronic) symptom of epilepsy that affects both adults and children.

A neurosurgeon carefully makes a surgical incision through the corpus callosum of your brain during a corpus callosotomy surgery. A band of nerve fibers called the corpus callosum joins the hemispheres, or two halves of your brain, and transfers messages from one side to the other. The spread of seizure activity is limited by a corpus callosotomy, which prevents seizure impulses from switching back and forth between the two hemispheres. This process is also known as brain-splitting or callosal sectioning.

Corpus callosotomy is a surgical intervention used to treat seizures that persist despite treatment with antiseizure medications. During this procedure, a precise surgical incision is made in the corpus callosum, a bundle of fibers connecting the two hemispheres of the brain. By severing this connection, the spread of seizure activity from one hemisphere to the other is restricted, helping to control seizures.

A corpus callosotomy is a surgical treatment for epilepsy. Epilepsy is a long-term (chronic) condition that causes repeated seizures in children and adults.

During a corpus callosotomy procedure, a neurosurgeon carefully places a surgical cut through the patient's brain’s corpus callosum. the patient's corpus callosum is a band of nerve fibers that connects the two halves of the brain (hemispheres) and relays messages from one side of your brain to the other. A corpus callosotomy stops seizure signals from crossing back and forth between the two hemispheres, limiting the spread of seizure activity. Other names for this procedure include callosal sectioning and brain-splitting.

A corpus callosotomy is one of several different types of epilepsy surgery. surgeons usually reserve this type of surgery for people with severe epilepsy.

What kinds of seizures are addressed by corpus callosotomy?

A corpus callosotomy is primarily effective in treating atonic seizures, also known as drop seizures or drop attacks. These seizures cause a sudden loss of muscle strength, leading to collapse or falling. Individuals experiencing atonic seizures are at an increased risk of injuries such as broken bones and concussions.

It's important to note that corpus callosotomy is not typically effective for treating partial (focal) seizures. These seizures originate in a specific focal region of the brain and usually do not spread to the other hemisphere. Corpus callosotomy is most beneficial when seizures initiate on one side of the brain and rapidly propagate to the other hemisphere.

A corpus callosotomy limits the spread of seizure activity by preventing seizure signals from moving back and forth between the two hemispheres. This process is also known as brain-splitting or callosal sectioning. This procedure is performed most often on children who experience irregular brain activity that spreads from one side of the brain to the other.

See a physician for a corpus callosotomy assessment if: You experience severe seizures on a regular basis that are unresponsive to medicines. Your seizures result in harm or have a major influence on day-to-day activities. Or Other treatment options (like vagus nerve stimulation or dietary therapy) have failed or aren’t suitable.

Your doctor may prescribe tests such as EEG, MRI, or PET to detect seizure activity in order to get ready for a corpus callosotomy. Before surgery, you will need to fast and modify your medication. Your provider will explain the procedure, risks, and obtain your consent.

A neurosurgeon uses precision instruments and a microscope to execute a craniotomy under general anaesthesia to access and surgically divide the corpus callosum. The dura and skull are then closed and secured. Sometimes the treatment is performed in two steps, beginning with the front part and, if seizures continue, maybe ending with a complete severing.

It usually takes 4 to 6 hours to perform the corpus callosotomy procedure, depending on the patient's state and the extent of the surgery.

  • Loss of coordination or balance difficulties
  • More partial seizures on one side of your brain.
  • Speech issues include difficulties speaking and/or comprehending the speech of others around you (aphasia) and difficulties formulating words and producing speech (apraxia)
  • Stroke
  • Brain swelling.
  • A disorder known as hydrocephalus occurs when water builds up in the brain

  • Reduced seizure frequency and severity
  • Improved safety and quality of life
  • Preservation of cognitive function

Following a corpus callosotomy, a hospital stay is necessary to monitor and treat any possible side effects, such as headaches or speech problems. The majority of symptoms go better with speech, occupational, and physical therapy. Recovery and ongoing seizure control are the main goals of follow-up care.

It works well for atonic seizures and other generalised seizures. Research indicates a 50–75% decrease in drop attacks and roughly 18.8% complete seizure freedom.
