Clinical Summary Before Treatment

Mahajabin Aktar, a 42-year-old woman from Bangladesh, began to experience severe and persistent headaches that started to become worse and worse. At first, she dismissed the headaches as stress or migraines, but eventually the pain began to interfere with her life. Eventually, the headaches became so powerful that they were coupled with blurred vision and sometimes vomiting. Her family began to worry and sought medical attention.

After conducting some brain imaging in Dhaka, it was revealed that Mahajabin was suffering from a suprasellar brain tumor, which is located near the pituitary gland, as well as moderate obstructive hydrocephalus (hydrocephalus is when fluid develops in the brain because there is a blockage causing pressure buildup in the skull).

The diagnosis was devastating for the family. They needed expert care as soon as possible. Through direct medical inquiry, they were put in contact with Arhama, a case manager at a medical travel company, who immediately began to work on the next steps.

The hospital initiated several preoperative assessments, including a repeat MRI, blood work, and anasthesia clearance. Dr. Kantha and his treatment team explained the procedure to the family in simple terms, ensuring they understood the risks, anticipated outcomes, and recovery time.

There were some critical complexities related to this case of hydrocephalus that needed to be addressed. It was imperative to relieve pressure in her brain, and it had to happen now, along with the removal of the tumor.

The Treatment and Recovery

Mrs. Mahajabin underwent excision of the suprasellar tumor with craniotomy under general anesthesia, and the procedure required skilled microsurgery due to the proximity of the tumor to the optic nerves and hypothalamus. The neurosurgical team, led by Dr. Kantha, utilised intraoperative navigation and other imaging modalities to assist in the surgery. The tumor was successfully removed, serving as the resolution of the hydrocephalus and subsequent obstruction.

The first 48 hours of the ICU stay were critical. The medical team closely monitored Mrs. Mahajabin for bleeding and infection, as well as changes in neurologic status. Gradually, she was titrated from sedation; importantly, her headaches began to improve, and her visual status began to stabilise.

The medical team provided instructions for Mahajabin and her caregivers on the medications to utilise, wound care and any concerning signals to watch for after discharge. Despite being a complex case, Mahajabin had a steady and smooth recovery course. She adhered to the rehabilitation program and was able to mobilise and communicate safely before hospital discharge.

After a total of twenty-three (23) days in Malaysia, Mahajabin was deemed fit to travel and discharged with a comprehensive discharge summary, all original MRI scans, bloodwork results, and surgical notes, as well as a rehabilitation and medication plan, and online follow-up appointments with Dr. Kantha. The medical travel team finalised her travel logistics and coordinated with a post-discharge care provider in Bangladesh to ensure continuity of care.

A Bright Future for Mrs. Mahajabin Aktar

Today, Mahajabin is back at home in Bangladesh, living life without the severe headache that plagued her life before. Mahajabin will continue her rehabilitation and undergo medical follow-ups, and she is gradually resuming her engagement with everyday life.
