Bone Marrow Transplant

A bone marrow transplant is a medical procedure in which the unhealthy/diseased blood-forming cells in the body are replaced with blood stem cells. These immature RBCs grow into white blood cells, red blood cells, and platelets. They are located in the bone marrow, a  soft substance that lines the inside of your bones. When they are fully developed, they leave the marrow and move into the blood.

To eliminate the diseased cells and marrow prior to transplant, chemotherapy (chemo) and occasionally radiation are used. Then you are given the healthy cells. BMT is not a surgical procedure. With the use of an intravenous (IV) catheter or tubing, the new cells enter your bloodstream. It is comparable to receiving blood or medication through an IV. The cells then enter your bone marrow from there. Recovery after BMT might take months or even years.

Frequently Asked Questions

A. Risks associated with bone marrow transplantation:

  • Graft-versus-host disease (GVHD): The donor cells attack the body of the recipient.
  • Infection: Infections can be severe and are treatable with various medications.
  • Bleeding: It can occur in the lungs, intestines, brain, and other parts of the body.
  • Anemia: It can take place as the blood counts are waiting to recover
  • Mucositis: It is inflammation and soreness in the mouth, throat, oesophagus, and stomach.
  • Gastrointestinal complications: Diarrhea, nausea, and vomiting may occur.
  • Organ damage: Kidneys, liver, lungs, and heart can be damaged.

Other complications include:

  1. Cataracts
  2. Delayed growth in children
  3. Early menopause
  4. Graft failure
  5. Pneumonitis
  6. Hepatic Veno-occlusive disease (VOD)

The majority of the time, the medical care team is in charge of managing the risk involved with BMT.

A. The length of stay in a hospital following the transplant of bone marrow (BMT) varies according to the diagnosis, transplant type, and other factors:

  • Autologous stem cell transplant: A hospital stay generally lasts around three weeks.
  • Allogeneic stem cell transplant: Hospital stay usually lasts around four weeks.
  • Pediatric BMT-A hospital stay is generally six to eight weeks but may be longer, based on other such factors as the type of transplant or the underlying disease.

Following discharge from the hospital, patients are expected to stay close within a range of one to three months. During this time, the attending doctors will continue to monitor the patient's recovery and watch for possible complications.

A. Yes, there are dietary restrictions after a bone marrow transplant, including:

  • Avoidance of unsafe food: One should not eat raw, undercooked meat, poultry, seafood, eggs, or meat substitutes. Unpasteurized dairy products, unwashed fruits and vegetables, or cold cuts should also be avoided.
  • Restriction of some foods: Avoid tart or acidic foods, strong spices, coarse foods, and food and drinks that are too hot.
  • Limitations of some foods: Limit red meat and processed meat, sugary drinks, and alcohol to <14 units/week.
  • Hydration: Drink at least 2 liters of fluid per day, and aim for 2-3 liters per day.

The healthcare team will check the weight regularly, and help one get enough nutrition.

A. After receiving a bone marrow transplant, one can travel, but it is important to speak with the doctor about safety precautions and when to avoid risk when traveling.

Though following a transplant, many specialists advise staying home for at least 100 days and restricting from using public transit or airplanes for at least nine to twelve months. Traveling is normally possible after the blood count is normal, and it usually takes a year after the transplant.

However, it is essential to take into account the potential risks associated with the destination and any necessary safety measures. For instance, there may be a higher risk of foodborne and waterborne germs in particular nations.

A. In India and other nations, bone marrow transplant recovery is similar. One of the most effective treatments carried out in India's healthcare system is bone marrow transplant surgery. 90% to 95% of cases are successful. A significant number of patients travel to India for bone marrow transplants because of the country's excellent healthcare system and high success rates. In India and other nations, bone marrow transplant recovery is similar. In comparison to other nations, bone marrow transplants are also very affordable in India.

A. Bone marrow transplant waiting times can vary widely across India depending upon various factors such as transplant types, donor matching, and certain institutions. Generally, waiting time is between weeks to months.

  1. Autologous Transplants: These usually have small waiting times since patients' stem cells can be collected and saved in advance for use.
  2. Allogeneic Transplants: Here the waiting time may be longer since usually a matched donor is to be searched through the registry. Finding a compatible donor may take time-sometimes several months.

Some hospitals may be more resourceful and perhaps faster than others, and that can affect waiting times.

The patient and the family should discuss this with the treating physician for the most precise time frames under each situation.
