Kidneys can be obtained from living donors and deceased donors. Living donor kidneys are most often reported to have a faster functional recovery and a longer functional life. Donor matching involves evaluating blood type, compatible tissue typing, and the donor's overall health.
The surgery involves placing the donor kidney in the lower abdomen and connecting the kidney's arteries and veins to the recipient's blood vessels. The transplant recipient's own non-functioning kidneys are typically left in place, unless they pose an issue for the recipient. The surgical procedure usually lasts for a total of 3–4 hours and is performed under general anesthesia.
Typically, patients will remain hospitalised for about one week to ensure adequate surveillance of kidney function and medication adjustments. Most patients return to a normal lifestyle around 6-8 weeks and feel much better as the newly transplanted kidney becomes functional.

A kidney from a living donor may function well for 15-20 years or even longer, whereas a kidney from a deceased donor typically lasts 10-15 years. The duration of a transplanted kidney's function is influenced by medication use, patient lifestyle, and access to follow-up care.
