Total Hip Replacement U/L: Symptoms, Classification, Diagnosis & Recovery

Unilateral total knee replacement (TKR), or unilateral total knee arthroplasty (TKA), is a surgical procedure to replace a damaged or worn-out knee joint with an artificial joint, or prosthesis, in only one knee. Here's an overview of this procedure: the unilateral (U/L) total hip replacement:

  • The damaged femoral head is removed and substituted with a metal stem inserted into the femur's hollow center. This stem may be secured with either cement or a "press fit" technique.
  • A metal or ceramic ball is affixed to the upper end of the stem, serving as a replacement for the removed femoral head.
  • The damaged cartilage surface of the acetabulum (socket) is excised and substituted with a metal socket. Sometimes screws or cement are utilized to secure the socket in place.
  • A spacer made of plastic, ceramic, or metal (known as a liner) is placed between the new ball and the socket to create a smooth gliding surface.

Classification:

Unilateral total hip replacement refers to the replacement of only one hip joint. It can be classified based on various factors:

  • Indication: Total hip replacement may be recommended for various reasons such as osteoarthritis, rheumatoid arthritis, avascular necrosis, hip fracture, or other hip disorders.
  • Type of Implant: The choice of prosthetic implants depends on factors such as patient age, activity level, and bone quality.
  • Surgical Approach: Common surgical approaches include posterior, anterior, lateral, or minimally invasive techniques.

Following are the signs and symptoms of total hip replacement:

  • Chronic Hip Pain: Persistent pain in the hip joint, often worsening with movement.
  • Stiffness: Difficulty moving the hip joint, particularly in activities like walking or bending.
  • Reduced Range of Motion: Inability to move the hip joint through its full range of motion.
  • Difficulty in Daily Activities: Struggles with tasks like walking, climbing stairs, or getting in and out of chairs due to hip discomfort.
  • Joint Instability: Feeling of the hip giving way or instability while walking or standing.

Before undergoing total hip replacement following diagnosis tests are performed to access the bones:

Clinical Evaluation: A thorough physical examination by a healthcare professional to assess hip pain, range of motion, and functional limitations.

Imaging Studies: X-rays, MRI, or CT scans may be performed to visualize the hip joint, assess the extent of damage, and evaluate bone quality.

Blood Tests: Sometimes, blood tests may be ordered to rule out underlying conditions such as infections or autoimmune diseases.

Unilateral hip replacement surgery recovery time varies from one patient to the other. The success of this surgery hugely depends on your ability to follow the instructions given by the surgeon's home care during the first few weeks after the procedure.

At the time of discharge, you will still have sutures and stitched under the skin. The doctor will remove stitches approximately two weeks after the surgery.

See that you do not wet the wound until it is sealed and dried. You can bandage the wound to avoid irritation from clothing or support stockings. The unilateral hip replacement recovery time is less as compared to bilateral surgery.

During the first few weeks of your surgery, perform the exercises the doctor advised. You may experience a little discomfort during activity and at night for several weeks. You will be able to return to conduct light activities within three to six weeks after the surgery.

Pain after hip replacement surgery goes away gradually. You may be given medications to manage pain after the surgery for a few days until it goes away completely.
