Capsule Endoscopy: Symptoms, Classification, Diagnosis & Recovery

Capsule endoscopy is a minimally invasive procedure that uses a small, wireless camera to capture images of the organs within the digestive tract. The camera is encased within a capsule, similar in size to a vitamin pill. Once ingested, the capsule traverses the digestive tract, capturing thousands of images along the way. These images are transmitted to a recording device worn on a belt around the waist.

This innovative technique provides visualization of the small intestine, a region that is challenging to access using traditional endoscopic procedures. Unlike conventional endoscopy, which entails the insertion of a lengthy, flexible tube with a camera through the throat or rectum, capsule endoscopy offers a less invasive and more comfortable alternative for examining the small intestine.

Classification

Capsule endoscopy can be classified based on several factors, including the technology used, the targeted area of the digestive tract, and the specific features of the capsule itself. Here's a breakdown:

Technology-based classification: It includes the following;

  • Wireless Capsule Endoscopy (WCE)
  • Tethered Capsule Endoscopy

Anatomical classification: It includes the following

  • Small Bowel Capsule Endoscopy
  • Esophageal Capsule Endoscopy
  • Colon Capsule Endoscopy

Capsule features-based classification:

  • Standard Capsule Endoscopy
  • Multifunctional Capsule Endoscopy
  • Diagnostic or Therapeutic classification

Signs and symptoms associated with capsule endoscopy are generally minimal, as the procedure itself is non-invasive. However, patients may experience some mild discomfort or side effects related to the ingestion of the capsule and the process of passing it through the digestive tract. Common signs and symptoms include:

  • Difficulty swallowing the capsules
  • Nausea or vomiting
  • Abdominal discomfort
  • Constipation or bowel obstruction
  • Incomplete passage of the capsule

Before undergoing capsule endoscopy therapy, patients typically undergo several diagnostic tests to assess their condition and ensure that capsule endoscopy is appropriate for them. These tests may include:

  • Medical History Review: The doctor will review the patient's medical history, including any symptoms they may be experiencing and any previous gastrointestinal issues.
  • Physical Examination: A physical examination may be conducted to assess the patient's overall health and identify any signs of gastrointestinal issues.
  • Blood Tests: Blood tests may also be performed to check for signs of inflammation, infection, or other abnormalities in the body.
  • Imaging Tests: Tests such as X-rays, CT scans, or MRIs may be ordered to get a better view of the digestive tract and identify any structural abnormalities or blockages.
  • Endoscopy: Traditional endoscopic procedures such as upper endoscopy or colonoscopy may be performed to visually inspect the esophagus, stomach, and colon for any abnormalities or signs of disease.
  • Preparation Instructions: Patients will be given instructions on how to prepare for the capsule endoscopy procedure, which may include dietary restrictions and bowel preparation to ensure optimal visualization of the digestive tract.

Post-capsule endoscopy recovery is simple and painless. Patients are advised to refrain from consuming clear fluids for a minimum of two hours after swallowing the capsule. A light snack or meal is permissible four hours post-capsule ingestion.

The capsule naturally exits the body within eight hours of ingestion, occasionally passing through the stool sooner. The specialist can review the abdominal images after the capsule has been expelled, whether that occurs within or after the eight-hour timeframe, depending on individual circumstances.
