What is a Follicular Lymphoma?

Follicular lymphoma is a type of B-cell non-Hodgkin lymphoma that develops from abnormal B lymphocytes, a type of white blood cell involved in the immune system. It is generally considered an indolent, or slow-growing, lymphoma, although its behaviour can vary between individuals. Follicular lymphoma commonly affects lymph nodes but can also involve the bone marrow, spleen, and other tissues.

What is the Importance of Timely Treatment?

Timely diagnosis and appropriate management can help determine the extent and biological characteristics of follicular lymphoma and guide treatment decisions. Because many cases grow slowly, immediate treatment is not always necessary. Some patients with asymptomatic, low-burden disease may be monitored through active surveillance, or watchful waiting.

What are the Common Symptoms of Follicular Lymphoma?

  • Painless swelling of lymph nodes in the neck, armpit, or groin
  • Persistent fatigue or weakness
  • Unexplained weight loss
  • Fever without an obvious infection
  • Drenching night sweats
  • Abdominal discomfort or fullness due to an enlarged spleen or lymph nodes
  • Recurrent infections in some patients
  • Shortness of breath or other symptoms when enlarged lymph nodes affect nearby structures

Causes and Risk Factors of Follicular Lymphoma

Causes

  • The exact cause of follicular lymphoma is not fully understood. It develops when genetic changes occur in B lymphocytes, causing them to grow or survive abnormally.
  • A characteristic genetic change involving the BCL2 gene on chromosome 18 is commonly associated with follicular lymphoma, although this change alone does not necessarily cause lymphoma.

Risk Factors

  • Increasing age
  • Certain abnormalities of the immune system
  • Immunosuppressive conditions or treatments
  • Some autoimmune diseases
  • Certain infections or environmental exposures may be associated with lymphoma risk, although their specific role in follicular lymphoma varies

Latest Research and Technologies in the Treatment of Follicular Lymphoma in India

  • Treatment of follicular lymphoma has evolved with advances in immunotherapy, targeted therapy, cellular therapy, and molecular diagnostics. Monoclonal antibodies targeting CD20 remain important treatment options. Targeted medicines such as selected PI3K-pathway therapies may be used in specific settings. For patients with relapsed or refractory disease, newer approaches include bispecific antibodies and CAR-T cell therapy, depending on disease characteristics, previous treatments, regulatory approvals, and patient eligibility.

Treatment options for Follicular Lymphoma

Immunotherapy : Anti-CD20 antibodies such as rituximab or obinutuzumab may be used alone or in combination with other treatments.

Cost Start From 1500Explore Options

Chemoimmunotherapy: Chemotherapy combined with an antibody may be considered for patients who require systemic treatment.

Targeted therapy : Targeted medicines may be considered in selected relapsed or refractory cases.

Cost Start From 1000Explore Options

CAR-T cell therapy : For selected patients with relapsed or refractory follicular lymphoma after previous systemic treatments, CAR-T therapy may be considered at specialised centres.

Cost Start From 80000Explore Options

Stem cell transplantation : Autologous stem cell transplantation may be considered in selected patients with relapsed or refractory disease, although its use depends on the clinical situation and availability of newer therapies.

Cost Start From 11500Explore Options

Clinical assessment:

  • Detailed medical history and physical examination, including assessment of enlarged lymph nodes, spleen, and other symptoms. Blood tests may evaluate blood cell counts, kidney and liver function, and other relevant markers.
  • Tissue biopsy: An excisional or core lymph-node biopsy is generally important to confirm the diagnosis and determine the lymphoma subtype and grade. Immunohistochemistry and other pathological tests help identify characteristic markers.
  • Bone marrow evaluation: Bone marrow testing may be considered in selected patients depending on the clinical situation and whether the results will influence staging or treatment.

Imaging Tests:

  • PET/CT scan: Helps identify metabolically active lymphoma and assess disease distribution and treatment response.
  • CT scan: Can evaluate enlarged lymph nodes and involvement of organs such as the chest, abdomen, and pelvis.
  • MRI: May be used when detailed assessment of particular organs or anatomical areas is required.
  • Ultrasound: Can help evaluate superficial lymph nodes or abdominal organs in selected cases.

Frequently Asked Questions

There is no fixed recovery period because follicular lymphoma is usually a chronic condition that may require long-term monitoring or treatment. Recovery after active treatment depends on the therapy used, disease response, side effects, and overall health.

Yes. Most people with follicular lymphoma can walk and stay physically active, especially when the disease isn't causing significant physical limitations. During treatment, you may need to adjust activity levels based on fatigue, blood counts, treatment effects, and medical advice.

Many specialised Indian cancer centres have access to advanced diagnostic and treatment technologies, including PET/CT, immunohistochemistry, molecular testing, immunotherapy, targeted therapies, and cellular therapies. Availability varies by hospital and by individual patient eligibility.

Many major Indian hospitals have dedicated international patient departments that assist with medical appointments, treatment coordination, travel documentation, accommodation, interpretation, and follow-up arrangements.

Consider a centre with experienced hematologist-oncologists, specialised lymphoma services, advanced pathology and imaging, access to appropriate systemic therapies, and multidisciplinary care. For relapsed or refractory disease, patients may also consider whether the centre provides cellular therapies such as CAR-T when clinically appropriate.

There is no single success rate for follicular lymphoma. Outcomes vary by stage, tumour burden, disease biology, age, overall health, treatment received, and response to therapy. Follicular lymphoma is often responsive to treatment, but it can relapse and may require long-term monitoring.

India has specialised hematology and oncology centres with doctors experienced in diagnosing and treating non-Hodgkin lymphomas, including follicular lymphoma. Experience and available treatments vary between institutions, so patients should review the specialist's lymphoma expertise and the hospital's treatment facilities.

Risks depend on the treatment used. Chemotherapy may cause fatigue, nausea, hair loss, low blood counts, and increased infection risk. Immunotherapy and targeted treatments can cause infusion reactions, immune-related effects, or other treatment-specific adverse effects. CAR-T therapy can cause complications such as cytokine release syndrome and neurological toxicities in some patients.

After diagnosis, consult a hematologist-oncologist to confirm the lymphoma subtype, grade, and stage and discuss whether immediate treatment or active surveillance is appropriate. Ask about the biopsy results, imaging findings, treatment options, expected benefits and risks, and whether a second pathology or specialist opinion is appropriate.

Follicular lymphoma does not typically cause permanent physical deformities. However, advanced disease or treatment-related complications can sometimes affect physical function, depending on which organs or tissues are involved.

Yes. Symptoms such as fatigue, enlarged lymph nodes, night sweats, emotional stress, and treatment-related side effects can affect physical, social, and emotional well-being. Supportive care, symptom management, physical activity, nutrition, and psychological support can help address these concerns.

Some follicular lymphomas can remain stable for years without immediate treatment, so active surveillance may be appropriate for selected patients. However, progressive disease can eventually cause symptoms, organ involvement, or complications. Rarely, follicular lymphoma can transform into a more aggressive form of lymphoma.

There is no established way to completely prevent follicular lymphoma because its exact cause is unknown. Maintaining overall health, avoiding tobacco exposure, following appropriate medical care for immune-related conditions, and discussing persistent or unexplained symptoms with a healthcare professional can support general health but cannot guarantee prevention.

Follicular lymphoma is more common in adults, particularly older adults, and occurs more frequently in some populations than others. The risk can also be influenced by immune-system disorders and certain other factors. Most people with these risk factors do not necessarily develop follicular lymphoma.
