Peroral Endoscopic Myotomy may be considered for appropriately selected patients with achalasia and certain other esophageal motility disorders. Before treatment, investigations such as upper gastrointestinal endoscopy, high-resolution esophageal manometry, and contrast imaging may be performed to confirm the diagnosis and characterize the disorder. During POEM, the endoscopist creates a submucosal tunnel and cuts the targeted esophageal and lower esophageal sphincter muscles to reduce resistance to swallowing.
