Portal hypertension without cirrhosis refers to a condition in which increased pressure within the portal venous system occurs, but the liver has not progressed to cirrhosis. This condition is due to impaired portal venous blood flow through the portal, hepatic, and/or splenic vessels. Portal hypertension without cirrhosis may be present alone or in conjunction with blood clotting disorders, infections, autoimmune disease, and/or vascular abnormalities. Individuals with portal hypertension without cirrhosis may have signs and symptoms such as splenomegaly, abdominal distention, GI bleeding, fatigue, or varices; in many cases, their liver functions are relatively preserved. Testing for portal hypertension without cirrhosis typically includes blood tests, Doppler ultrasound, CT or MRI scans, endoscopic procedures, liver stiffness testing, and sometimes liver biopsy (to evaluate for underlying causes or complications).
